New Patients
If you have an
upcoming new patient appointment for cardiac consultation,
the following information will help make your first visit as
comfortable and informative as possible. If you are
scheduled for a stress test, echocardiogram or other
outpatient procedure, please see our Procedures section for
further information. Finally, if you are interested in
seeing us as a new patient but have no appointment, please
see your primary care provider or call (360) 256-2640 during
normal business hours for more information.
- In
an attempt to provide a smoother and timelier check in,
you will receive a packet of forms by mail prior to your
visit. Please fill them out completely and bring them
with you to your first appointment. If for any reason
you do not have these forms, you may download and print
them now.

You will be asked to arrive 20 minutes before your
appointment time to allow for a small amount of further
paperwork that must be completed at our office.

Bring your medication. In fact, it is most helpful if
you bring the actual bottles of all current medications
to all of your appointments. But it is especially
important at your initial consultation.
- Our
providers are occasionally called out of the office or
run behind due to urgent, unscheduled needs of
patients. Our staff makes every effort to contact you
as soon as we are aware of the interruption in the
schedule in order to adjust your appointment
accordingly. This may result in asking you to see
another provider or in the rescheduling of your
appointment. Your patience and understanding are
greatly appreciated.

Conversely, if conflicts force you to reschedule your
visit or if you are running late, please let us know as
soon as possible. It is a simple courtesy to others who
are patiently waiting to see us

We have already requested pertinent medical records from
your referring provider. If you have any additional
medical records, pictures, device information, etc. that
is pertinent to your cardiovascular history, please
bring them with you to your appointment. This
information may help us provide you with a more
knowledgeable and comprehensive opinion and treatment
plan during your initial visit.
